To complete the license you still need:

• TWIC card  
• Completed application including three letters of recommendation 
• Sea service forms 
• Sign an oath (US CG form) 
• Pay the Coast Guard (postal money order or, pay.gov) -application fee $100   issuance fee $50
• Certificate(s) of Completion from this class

419-418-6010 and ask for in the original license application. Speak clearly and leave your address.
